Unexplained Infertility Diagnosis: Proactive Lifestyle Changes and Next Steps
You’ve been trying to conceive for over a year without success. You’ve done fertility testing — hormone panels, ultrasounds, semen analysis, and more — only to get a diagnosis of unexplained infertility.
Everything appears normal on paper, but you haven’t gotten pregnant. It’s normal to feel confused, helpless, and wonder what you’re supposed to do next. That’s where we can help. Our team at OB-GYN Associates of Marietta specializes in infertility care in Marietta and Woodstock, Georgia.
June is World Infertility Awareness Month, and now is the perfect time to learn more about what unexplained infertility really means and, more importantly, what proactive steps you can take to move forward.
Understanding unexplained infertility
Unexplained infertility is diagnosed when comprehensive fertility testing doesn’t reveal any obvious cause for your inability to conceive. Ovulation appears normal, your fallopian tubes are open, your uterus looks healthy, and your partner’s sperm count and quality fall within normal ranges — but despite everything seemingly functioning properly, you haven’t gotten pregnant.
It can be a frustrating diagnosis, but you’re not alone. Unexplained infertility accounts for approximately 30% of all infertility cases, making it one of the most common diagnoses fertility specialists encounter.
Despite the “unexplained” label, it doesn’t mean nothing is wrong. It simply means current testing methods haven’t identified a specific, treatable cause. Subtle issues with egg quality, sperm function, fertilization, embryo development, or implantation may exist but remain undetectable with standard diagnostic tools.
What happens after an unexplained infertility diagnosis
An unexplained infertility diagnosis isn’t the end of the road, and we don’t just hand you a diagnosis and send you on your way. We partner with you to develop a comprehensive, personalized plan that gives you the best chance of achieving pregnancy.
Comprehensive fertility testing
We start by making sure your fertility testing was truly thorough. We verify that all baseline evaluations are complete, including detailed hormone assessments throughout your menstrual cycle, advanced imaging to examine your uterine cavity and ovarian reserve, comprehensive semen analysis, and evaluation of your ovulation patterns through tracking and blood work.
Sometimes, a diagnosis of unexplained infertility changes when we dig deeper or use more advanced testing methods. And if we find a cause through additional testing, we can recommend treatment to improve your chances of conception.
Proactive lifestyle changes
While we work on your medical treatment plan, you can also start making some lifestyle changes that optimize your fertility potential and overall health right away. We can discuss changes such as weight management, a healthy diet, stress management, and prioritizing quality sleep.
Achieving and maintaining a healthy body weight can improve ovulation and conception rates for some women. Being either underweight or overweight can impact your fertility, and even a 5-10% weight change in either direction can make a difference.
A fertility-friendly diet is rich in antioxidants, whole grains, lean proteins, and healthy fats. Limit processed foods, trans fats, and excessive sugar. Limit caffeine to less than 200 mg daily (about one 12-ounce cup of coffee) and eliminate alcohol consumption while trying to conceive, as both can negatively impact fertility.
If you or your partner smokes, quitting can significantly improve your fertility. Finding healthy ways to manage stress, such as yoga, counseling, or other techniques, can help keep stress hormone levels down. Similarly, getting seven to nine hours of sleep nightly helps keep reproductive hormones balanced.
Medical treatment options
Beyond lifestyle optimization, we can discuss medical interventions that can help you overcome unexplained infertility. For example, medications like clomiphene citrate or letrozole can stimulate your ovaries to produce multiple eggs per cycle, even if you’re already ovulating regularly.
More eggs increase your chances that one will be fertilized. Combined with careful monitoring and timed intercourse, this approach offers a relatively low-intervention first step.
Intrauterine insemination (IUI) can be another successful option, and it involves placing specially prepared sperm directly into your uterus during your fertile window. This procedure bypasses potential barriers in the cervix and shortens the distance sperm must travel to reach your egg.
In vitro fertilization (IVF) is often the most effective treatment for unexplained infertility, because it bypasses many potential subtle issues. We retrieve eggs from your ovaries, fertilize them with sperm in our laboratory, culture the resulting embryos for several days, and then transfer the highest-quality embryo into your uterus.
An unexplained infertility diagnosis is frustrating, but it’s far from hopeless. This Infertility Awareness Month, know that we’re here to help you create the best possible conditions for pregnancy to occur. Schedule a consultation to start discussing the next steps toward building your family.
